Q: |
I Am sending another question after the response to my last question concering IBC. I mentioned that there was swelling and redness and fever localized to the breast where I seven years ago had cancer and was treated with lumpectomy, chemo and 40 treatments of radiation. I am seeing a breast specialist and he first did mammogram, ultrasound which were negative for tumors however, he said to wait a month and then he would possibly do a biopsy. I went back expecting to have a punch biopsy but he said he needed a larger section of the breast to get a proper diagnoses. The redness has turned to pink but the tenderness and swelling is still present also there is some distortion of the nipple and areola area with raised like places. my question is if there is IBC what treatments are used for that if I have already had cytoxan, adriamycin and 5FU with 40 treatments of radiation?
|
| |
A: |
possibly a taxane like taxol or taxotere. pursue this... |

Q: |
Hi, April 2004 (aged 44) I had an inner upper quadrant IDC of left breast, 1.2cm, node negative, ER+, radiotherapy and chemotherapy (AC x 4). I have been well since then. Have just found a small hard lump in the other (right)breast. It is like a grain of rice. I am due my annual followup very soon but am wondering what are the chances that this is another cancer? I am on tamoxifen and wonder if that would exclude the possibility of fibroadenoma. I'm an ex nurse and not scared but I prefer knowing. Many thanks. |
| |
A: |
the risk for breast cancer in the contralateral side averages 5%. be optimistic. |

Is further testing or treatment needed?Thank you. |
| |
A: |
sometimes MRI is done. they need to determine if a tumor is causing this. |

A: |
you need to pursue this. so call your doctor about getting a diagnostic evaluation done in breast imaging to determine what this is. they will probably do a mammmogram and ultrasound and possible core biopsy. |

the cause of mastitis sometimes is never determined. what is important though is to ensure that it IS mastitis and not something more serious. |

Q: |
hi, i am a 42 year old overweight male. 3 months ago i noticed a pain near my left nipple. It only hurt if i rubbed it and mainly through my clothes. Both my breasts are flabby and there are a number of fatty lumps in both breasts mainly about the same areas. I have visited all 3 of my docs who all believe there is nothing sinister. There is no pain unless i touch it and as i said it is mostly when rubbed through my clothes. The pain seems to be easing slightly but if i touch it alot it seems to flare up again. What can it be please |
| |
A: |
ask them about doing an ultrasound of the area to see if the radiologist can determine what is causing this pain. |

Q: |
What is a papable nodule in the breast and what does it mean? |
| |
A: |
it means a lump that can be felt with your fingers on breast exam. so the next step is further diagnostic evaluation to determine what it is. remember too that most lumps end up being benign so don't panic yet! |

Q: |
Good day! What does itchy nipple means?? does it mean that I have a breast cancer?? Also my breast sometime hurts for no reason at all then soon it will fade away?? Due to the itchiness often scratch it then the skin in my areola is starting break then i sometime peel it off and see the pink skin. i dont feel any lump in my breast. Is this the sign of a stage one cancer?? or just a bacteria that only needs plain medicine. im just an 18 years old student. please help. THANK YOU |
| |
A: |
probably not cancer.... probably more related to hormonal changes. but you need to see your gyn doctor so that you can get this taken care of because scratching can cause an infection. |

Q: |
I just had a papillary cyst/tumor removed. It was discovered that I have atypical ductal hyperplasia and it was suggested I go on Tamoxifen for five years. (I am 41). I do not want to go on Tamoxifen! This is terrifying to me. I thought atypical ductal hyperplasia is considered a "benign" breast disease but then it is suggested I go on a cancer med? I am freaking out. I think I would rather be told I have cancer now and here's what I need to do rather than be left with the choice to decide if I want to take a drug for five years that has awful side effects or not take it and then hate myself forever if I do get cancer that could have been prevented had I taken the drug. This is an awful place to be. I don't know what to do. I will have to worry forever about getting cancer now. I just want to have bilateral mastectomies now and get this over with. How am I supposed to live life worried constantly since now I am in this "super high risk category"? What have other women done in my situation that have been diagnosed with this? How bad is this condition really? Help! |
| |
A: |
ADH is a risk factor for breast cancer but not cancer itself or premalignant cells either. tamoxifen does reduce risk however by 47%. no one can make you take it-- you need to be well informed and think this through. so consider a formal second opinion about it with a high risk specialist. |

Q: |
I have a white pimple-looking place on the tip of my nipple for about 6 weeks. No pain and no discharge and has been kind of rough and scaly. I had DCIS in the other breast with mastectomy and reconstruction 5 years ago. I am 50 years old, have never been pregnant, had hysterectomy 4 years ago and no hormone replacement therapy. What could this be? |
| |
A: |
the scaliness is a little worrisome... time to see your breast surgeon again and get this checked out. hopefully it is something benign but pagets disease of the nipple needs to be ruled out based solely on your deion of course. |

Q: |
I just had my first mammogram at 37. The results came back as an area of asymmetric breast tissue is present measuring 25 mm at 12 0'clock. My aunt on my mothers side died of breast cancer at the age of 40. They are sending me for a spot compression test. Is this the proper next step. What concerns me is how large the area is. When I search I always find exapmles with areas 5 mm and less. Thank you |
| |
A: |
not that unusual to see a 2.5cm mass. yes, additional spot compressiong and possibly ultrasound will be the next steps. this may lead to a biopsy. |

| |
A: |
if the spot were confirmed to be cancer, and is invasive disease yes it has the ability to travel through the lymph nodes or blood vessels elsewhere. |

Q: |
I am 35 years old and have found a half-moon shaped raised ridge in my right breast that I am really worried about. It is approx 3 inches up from the nipple on the outer side. It doesn't feel hard or sore. It feels like it is closer to the muscle underneath than actually in the breast but I'm not sure. I have also found a soft movable lymph node half way down my rib cage on the same side. Now the ribs on that side are very sore to touch and the area feels uncomfortable. I'm not sure if this is because I have been feeling this area about 50 times a day I'm so anxious about it. I have also had shoulder pain and arm pain on and off. My arm feels heavy and the muscles sore when I move it. I also have very lumpy breasts and I'm not sure what's normal and what isn't. Do you think this half-moon shaped ridge could be a tumour? Are most breast tumours hard and round or can they be any shape? I am so afraid to go to a doctor. Please let me have your advice. Thank you. |
| |
A: |
we usually think of cancer being hard and rigid but not always when it comes to breast lumps. without seeing you no one can guess. you need a clinical breast exam and a mammogram. so pursue getting this done so this can be deciphered. |

Q: |
Hi, I'm 32 years old and from have fibercystic breast. They are painful at times and extremely heavy. Lots of neck/back pain and headaches. They are 36D and I'm pretty small framed.Years ago I was sent to 2 surgeons to have checked after mammograms. they said at the time I was too young to start "cutting" on and to back off of caffeine, etc. Also, they stated that basically my breasts were just one big mass of cysts and if they were to drain them I'd probally be left with nothing. Anyway, all theses years later and 2 children later I'm ready to focus on me for a change. I've talked to several people that have had reductions but noone that has fibersystic like me. Can I have a breast reduction? Hoping that things have advanced since I was younger. Just wondering how they would seperate the cysts and I'd be ok down the road. We just switched insurance to BCBS and I have unlimited chiropractor files so I know I can probally get approved once I start this just wondering if I can get a heads up if this can be done. ...By the way, each of my breast literally weighs at least 5-7 lbs .each. Thanks so much,
|
| |
A: |
A large majority of women have fibrocystic breasts and it simply means they are lumpy and bumpy-- not a precursor to cancer at all and many have had breast reduction as augmentation done. |

Q: |
I am 33 years old with a 10 year old son and in fairly good health condition (meaning I have high blood pressure, but it is under control). I had my annual GYN exam earlier this year. However, a few days ago I noticed that my left breast is slightly longer than my right breast. I don't recall my breast ever being even in size, however I have never noticed this. My GYN exam did include a clinical breast exam (which came out okay) and my thyroid test came back okay.On my maternal side of the family, there is no history of breast exam. My breast do seem to increase in size when I gain weight, other than that, I have no problems. Is this a early warning sign of cancer? |
| |
A: |
first, paternal side of the family is equal to maternal side of the family so when looking at breast cancer don' just count the mother's side. that said, any time there is a change in the side or contour of the breast it needs to be further evaluated. so call your gyn for a clinical breast exam again. |

Q: |
Hello .. I am 43 years old and I have been having a painful tearing sensation in my left breast under my arm for about 1 month now. It started when I would roll over to my left side at night .. I would get the pain .. I usually sleep with my arm up .. It has gotton worse each night and for the past few days the pain lingers after I get up in the morning. I had a mamogram in Nov that came back fine. But now I am worried what could be causing this pain. Please help, especially since it is getting worse each time. |
| |
A: |
a clinical breast exam is needed to decipher this so call your doctor about it and get seen. he might decide to do an ultrasound as well. |

Q: |
Two days ago I went to my gyn doctor for check up . he performed a breast exam and foun a solid mass 4x3cm. he send me mamogram and a surgeon. do you think he can be wrong and this is not a lump? since i have a mamogram 1 year ago and iam 39 years old |
| |
A: |
if he felt something then you need to pursue this with diagnostic evaluation in a breast imaging setting. don't assume that a lump means cancer. |

Q: |
Hello. I am a 40 year old mother (Ashkanazi descent). I gave birth to a child at age 37 (breast fed for 13 months). My grandmother had breast cancer in her 30s (died at 75 of heart disease). In my right breast (upper left side above rib) is a small swelling. It's as if the rib feels more prominent on that side than on the left side. However, it's difficult to actually feel a lump. My doctor wants to see me back in a month but told me not to worry. Would you have any thoughts? Thank you for your time. |
| |
A: |
He may be waiting to see if it is being caused by hormonal changes. You have described several risk factors however for developing breast cancer in the future so consider being evaluated anyway but an oncologist who specializes in high risk women. |

Q: |
Today I was seen by the Breast Care clinic for palpable masses on both my right and left breast that ultrasound and mammogram 2 weeks ago could not ascertain. The expereinced-seeming doc thought, after palpating it and listening to the fact that they hurt, the bigger one most of the time and the smaller only in the week before my period, that they were fibrocystic changes and that I could do 1 of 3 things. (I need help with the decision.) 1 - monitor with self checks for next 6 mos then have another ultrasound to see if they have grown or changed, 2- biopsy although at only 1 cm for the largest one, he thought he might not get an adequate sample, 3 - surgically remove the largest (right) lump and have the lab analyze it. If it were okay, then the other should be too. If not, then they'll check/remove the other as well. I'd like to go the aggressive route even though doc thinks there is only a 0.5% chance it's cancerous. My reasons are 1 - I have a 3 year old for whom I'd like to be there as long as possible, 2 - I would like to have another child, but want to be sure all is okay before getting pregnant (I'm soon to be 40 so time is of the essence), 3 - my mom had aggressive breast cancer diagnosed a year ago (nothing palpated in routine exam in Nov, but by Feb was already 3.5 cm) stage 3 breast cancer diagnosed and treated (mastectomy and 2 types of chemo) and her lump, like mine, hurt, so I gather pain is not really a reason to not worry. The thing is, I'm part of an Anesthesiology department and know even the simplest surgery with general anesthesia can be risky business. My 3 year old will have a tonillectomy in 2 weeks, just when they could 1st schedule to remove the fibroadenoma. One piece of info I lack is about the recovery from the "small incision" surgery to remove the 1 cm lump in the 7 o'clock position of my right breast? Would I need to not lift my 40 lb son for a while, while he's recovering from tonsillectomy? Perhaps I should just wait until he's all recovered. What do you recommend? |
| |
A: |
ask about using propofal anesthetic-- 15 second half life. no lifting for 7 days after excisional procedure is the general rule. understandable your anxiety with your mom's history. |

Q: |
I am 30 years old and pregnant with my third child. I am about 33 weeks and am having pain in my right breast where my lymph nodes are. This pain has come and gone frequently during this pregnancy. Sometimes it is sore, and sometimes it is not. It seems to be worse at night and in the morning. Is having breast soreness in this area common during pregnancy?? I do think I feel a lump but am wondering if it is my lymph nodes enlarged or infected. Please advise. Thank you! |
| |
A: |
hormonal changes can cause all kinds of breast sensations. it would be good to tell your OB doctor about this though so he can examine your breasts and ensure all is well. congrats on your pregnancy. |
