| 02/17/2005 | I have a family history of breast & ovarian cancer (both grandmothers w/breast, 3 paternal aunts w/breast, mother w/ovarian). I am 40 years old and had a lumpectomy & sentinal node biopsy 01/18/05. My nodes were clear. They removed a .5cm invasive ductal carinoma, er/pr positive, her2neu negative, and some areas of DCIS totaling 1.3cm. Two margins on DCIS areas not clear on pathology. Anticipated treatment was re-excision, radiation, 5 yrs tamoxifen, no chemo. Had re-excision on 02/08/05 and found more DCIS, margins still not clear. Pathology report says I am multicentric. Am getting mixed signals from surgeon & oncologist on whether to try a second re-excision or just do a mastectomy. What factors are important in this decision? | ||

| 02/17/2005 | Several factors for you--- your breast is trying to tell you something.. that it has what sounds like extensive DCIS. Equally important though is to pursue genetic testing. you have a gene and if so you might want to consider mastectomy with reconstruction even unrelated to your present difficulty with getting clear margins. if you wish to come to us for evaluation and possible treatment just call 443-287-2778. at some point the amount of tissue removed compromises the cosmetic result of a lumpectomy. add radiation to that and more compromise usually. but think about genetics... your family history is significant. | ||
